Film production company Andalusian Dogs LLC is conducting a survey of Traverse City, Michigan, and surrounding areas for use in an upcoming documentary.
They are searching for people who've been diagnosed with the following diseases:
Guillain Barre Syndrome
Amyotrophic Lateral Sclerosis ( Lou Gehrigs disease)
Multiple Sclerosis
Fibromyalgia
Chronic Fatigue Syndrome
Alzheimer's Disease
Lyme Disease
Parkinson's Disease
Autism
Non Hodgkin's Lymphoma
Please include your name, age, gender, type of diagnosis, address and
how long you've lived in the area. Information obtained will not be
used for solicitation, sold to a third party or reprinted. Names and
addresses will be kept confidential. Names are required to keep survey
honest and addresses will be used to track diagnostic trends in
certain areas. Information will be used solely for statistical
research and presentation.
